Due to the latest Coronavirus pandemic a large segment of the population is unable to get access to quality health care services. For the past several years we've been bringing care to patients that are homebound or otherwise unable to travel to a clinic.
We provide primary care services to those patients who do not have a current PCP and can coordinate interim care should your existing PCP be unavailable. We can do a medical encounter similar to those you experience in a clinic setting. We coordinate laboratory services, prescribe medications, and coordinate to specialists and other healthcare professionals as necessary. Our services are available to those who choose not to seek medical care in a clinical setting, and prefer to receive services inside their own home.
Doctor’s House Calls, along with Signature Healthcare, and Multicare Home Health provides state of the art wound care services! As SWCA approved providers, our team works in conjunction with Surgical Wound Care Associates and it’s physician directed training protocols. Together, we bring a synergistic approach to wound care that optimizes outcomes, increases patient satisfaction, and expediates wound healing time.
We provide treatment and oversight for patients recently discharged from a hospital, LTACH, and in some situations Skilled Nursing Facility. Transitional care refers to the coordination and continuity of health care during a movement from one healthcare setting to either another or to home, called care transition, between health care practitioners and settings as their condition and care needs change during the course of a chronic or acute illness
